Step 3. On the next page, select your state and district. Then, use your Aadhaar number to check your eligibility for the card. Here, you will see your family and card status.

Step 4. If the card status is "Not-Generated," you can apply for the card by clicking on "Apply Now."

Step 5. Now, you will need to fill in your personal details, name, date of birth, and family information. You will also need to upload a scanned copy of your Aadhaar, ration card, or other relevant documents.

Step 6. Finally, you'll need to verify your identity. To do this, generate an Aadhaar OTP on your Aadhaar-linked mobile number. After submitting this OTP, review your details and complete the Ayushman Card application process.
