How to find Ayushman Hospital
First of all, go to the official website of Ayushman Bharat Yojana (pmjay.gov.in).

After this click on the option of 'Find Hospital'.

Now enter your state, district, and hospital (government or private).

After this, you have to choose the disease for which you want to get treatment.

Now select PMJAY in Empanelment Type.

Enter the captcha code shown on the screen and click on Search.

After this, you will see the list of hospitals listed in Ayushman Yojana on the screen.

In the listed hospitals, you will also see which disease is covered in which hospital.

How to apply for an Ayushman card
The process of applying for an Ayushman card is quite easy. You can apply for it offline as well as online. To apply online, you have to visit the website of Ayushman Bharat Yojana. Whereas, for offline applications, you have to contact the hospital or health center of the state or district.
