While this young woman was living her normal college life, she was diagnosed with a 4 cm follicular neoplasm in the left lobe of her thyroid. The diagnosis worried her, but the prospect of a permanent neck ulcer after conventional thyroid surgery made her even more uneasy.

After a detailed examination, the Head and Neck Surgical Oncology team of HNCII recommended a modern and less invasive surgery called Transoral Endoscopic Thyroidectomy Vestibular Approach (TOETVA). In this technique, the thyroid gland is removed through a small incision on the inside of the lower lip, eliminating the need for an external incision on the neck.

This highly specialized surgery was successfully performed on 27th December 2025 under the guidance of the surgical team of HNCII. This successful procedure makes HNCII one of the few centers in India to offer this state-of-the-art scarless surgery to select thyroid patients. Compared to traditional thyroid surgery, TOETVA involves three small incisions inside the mouth. Since these incisions are on the mucosal surface, they heal naturally and leave no visible ulcer.

The outcome of the surgery was very satisfactory. The patient’s condition improved rapidly. Her neck was left with no ulceration, minimal tissue damage, and no effect on her voice or laryngeal function. After a short rehabilitation period that included oral hygiene and a soft diet, she confidently returned to her college life.

Dr Sultan Pradhan, Surgical Oncologist, HNCII said, “Surgeries like TOETVA are revolutionizing the field of thyroid treatment. This method offers the same cancer safety as conventional surgery, but avoids visible ulceration. This result can be very important, especially for young patients. He added, “For many patients, it is not just a matter of aesthetics. A sore on the neck can be a constant reminder of illness and treatment. With selective minimally invasive techniques like TOETVA, we not only address medical issues, but also help preserve patients’ mental health, confidence and quality of life.”

Speaking about the wider importance of this process, Dr. Pradhan explained that while TOETVA is not suitable for every thyroid patient, it is an effective option for patients with small thyroid nodules, benign disease or low-risk thyroid cancer and a strong desire to avoid ulceration cosmetically. Although TOETVA requires advanced laparoscopic skills and very careful patient selection, this method is making a big difference in the treatment experience of patients by combining effective treatment and excellent aesthetic results.

Today, the 19-year-old is back to her daily life and is confidently pursuing her education and future goals. Her example shows that innovations in surgical oncology are not limited to cure but also focus on preserving patients’ dignity, self-confidence and overall quality of life. This successful surgery highlights the growing role of scarless surgery technology in India and gives a ray of hope to patients who want to keep their personality and self-esteem intact along with effective treatment.
