If you want to add your name in ration card, then you need these documents:-
- If your child's name has not been added to the ration card yet, then you can get it added, for which you first need the child's birth certificate. Keep in mind that this is the most important document for adding the name to the ration card. You can get it from Nagar Nigam, Nagar Palika or Gram Panchayat

- If you want to add your child's name to the ration card, then the second document for this is the head's photo. You need a passport size photo of the head of the household (in whose name the ration card is)
- To add the name in the ration card, you also need the Aadhar card of the child

Step 1
- If you also have all the necessary documents, then now you have to visit the official website of the food supply department of your state
- For example, if you are a resident of Uttar Pradesh, then you have to visit the official website of the Food Supply Department here https://fcs.up.gov.in/

- After this, you have to click on the option 'Add name of new member in ration card'
- Then the application form will appear in front of you where you have to fill all the information about the child
- Also, attach the required documents and submit the application by paying the application fee.
