It Ends with Us star announces tragic cancer diagnosis after film success

The beloved author Colleen Hoover, who was the author and producer behind Blake Lively's hit rom-com It Ends with Us film, has revealed she is battling cancer. On Monday (January 12), the author took to Instagram to share a worrying health update with her 1.9million followers. Sharing a snap dressed in a blue hospital gown, Hoover pulled her hair up into a messy bun as she grimaced in the mirror selfie.

The producer, who is set to have another of her novels adapted into film, gave fans an insight into her treatment as she revealed she was undergoing her "second to last day of radiation". She added: "I wish I could blame my hair and facial expressions on @Texas.Oncology, but they've been great. Hope you never need them, but highly recommend them."

Hoover is the brains behind both It Ends with Us and Regretting You - both of which have gone on to become Hollywood blockbusters. But her latest film, starring Lively and Justin Baldoni, was overshadowed by their backstage drama.

Backstage on the explosive film set led to Lively filing a legal complaint against Baldoni and his production company for sexual harassment and creating a hostile work environment.

Baldoni vehemently denied the allegations and later filed a $400million countersuit against his co-star and her husband, Ryan Reynolds, claiming they defamed him and tried to launch a smear campaign.

Much of this countersuit was later dismissed by a judge. The case is slated to return to court for a January 22 hearing, with a May 18 trial date looming ahead.

Hoover first mentioned a health issue late in 2025, when she missed a major milestone ahead of the release of her other film, Regretting You. She wrote in October: "I'm super bummed but am having an unavoidable surgery and can't travel for a while."

She added that she was "so sad to miss this movie release and premiere, but so grateful to all the actors and the team who put this together." In a separate Facebook post, Hoover revealed that the cancer had been "removed through surgery" and that it would be further treated with radiation.

She explained: "While it felt huge and scary for a bit, and I had to miss out on the Regretting You premiere, and some other important career and personal moments, I just wasn't ready to share with anyone until I knew what the outcome would be."

After sharing the lengthy update, Hoover urged fans to "listen to your body" and to investigate if something felt "off". It Ends with Us earned a staggering $148million in domestic box office and $350million globally.

The successful film, which also starred Jenny Slate, Hasan Minhaj, Brandon Sklenar, and Kevin McKidd, was based on her 2016 novel. The storyline centred on a toxic relationship between Lily Bloom (Lively) and Ryle Kincaid (Baldoni).

According to the film's logline, Lily "overcomes a traumatic childhood to embark on a new life" when "a chance meeting with a neurosurgeon sparks a connection, but Lily begins to see sides of him that remind him of her parents' relationship."
